Application Process
NEW INDIVIDUAL PRESBYTERY OFFICE INSURANCE
Application Processing Procedure - 2011
1. For an application:
Go to application section, print and fill out
2. Fax completed applications to:
Affiliated Insurance Services, Inc., Attention: Daryl Overcash

3. Application will be forwarded to underwriting for review and pricing immediately upon receipt
from the individual Presbytery.
4. When final pricing is received by Affiliated Insurance Services a quotation and coverage
will be mailed to the Presbytery at the address shown on the application. A return
envelope is provided for your premium check. Receipt of your check will serve
as binder for your coverage.
5. When premium payment is received from the Presbytery a paid receipt
will be mailed to the Presbytery confirming coverage.
6. A binding request will be sent to underwriting immediately upon receipt
of premium payment.
7. When we receive the policy from the insurer, the Presbytery's copy will
be immediately forwarded to the insured Presbytery.
